All dossiers must conform to these requirements. Any that do not will not be considered by the committee.

2. Executive summary (1 page) of the dossier signed by the dean or chair, as appropriate. The most useful summary does more than list the contents and reiterate the letters. In its own words the summary illustrates why the Teaching Awards Committee should give an award to the nominee. Thus it makes a clear makes a clear, persuasive case for the nominee. For example, the executive summary might identify the main reasons for the nomination and/or summarize to the nominee’s strengths and talents.
3. Nominee’s abbreviated curriculum vitae (2 pages)
4. Nominee’s reflective and personal statement about teaching from the nominee (1 page). The committee is looking at how nominees articulate their beliefs about and commitment to teaching and learning.
5. Student evaluation data, including qualitative comments and overall ratings, with comparison to relevant courses (2 pages). In order to make these data as equitably comparable across schools as departments as possible, the committee requires that nominators use the Excel form to provide the requested data, preferably from the University-wide on-line system. The form includes a sample entry; here are details about the required information:
Provide 3 years of data, including all the UVA courses the nominee taught during those years. Use the “Quantitative Data” tab to provide average ratings; use the “Qualitative Comments” tab to provide sample student comments. Details:
- For each Course, identify the term (fall=F, spring=S, summer=Sum, J-Term=J); year; course number; # of credit hours; short course title; and course level (undergraduate=U, master’s level=M, PhD or doctoral level=P).
- For Class Size, give the maximum possible enrollment, the actual enrollment, and the number of students who completed the end-of-course evaluation. Excel will figure the percentages that will appear in the gray boxes.
- For Ratings:
- Give the maximum rating on the scale used (i.e., are the ratings on a 5-point scale? a 4-point scale?).
- Note the group used for comparison. If the course ratings are compared to all courses in the department, indicate this with “D.” If the course ratings are compared to other department courses at the same level, indicate this with “L.” If another group of courses is used for comparison, use that group and explain or define it.
- Give the nominee’s and then the comparison group’s mean (average) rating in response to the Provost’s-level overall questions:
- Overall, the instructor was an effective teacher. (“Teacher Ratings”)
- Overall, the course was a worthwhile course. (“Course Ratings”)
- For Qualitative Comments, use the second tab.
- Provide up to nine comments from students in 3 or 4 different courses over the three-year time period.
- Identify each course by term, year, course number, and course title
6. Six one-page letters of support in this order, in 12-point font, Times New Roman, with margins of 1” at top and bottom and 1.25” at sides):
- 1 from the nominator (1 page)
- 2 from faculty colleagues (1 page each)
- 3 from students (1 page each)
